Information you submit
The call request form asks for your first name, last name and phone number. When you select “Submit,” those details are sent to Village Mortgage through Google Apps Script and stored in a restricted Google Sheet, together with the time received and a submission identifier.
How we use your information
Mike and Juli use your details to respond to your call request. The Sheet is not public. This form does not run a credit check, make a lending decision or request payment. Do not submit Social Security numbers, account numbers, tax returns or other financial documents here.
Website services
Google processes the submitted information to operate the form connection and spreadsheet. The website host processes requests needed to deliver the site, which may include technical information such as IP addresses. No advertising analytics or browser storage has been added to this form. Your details remain in the open page while you complete or retry the form; reloading or closing the page clears that local form state.

To ask about information you submitted, request a correction or deletion, or ask about applicable retention practices, contact Village Mortgage at 801-627-8888. Retention and deletion are subject to applicable recordkeeping requirements.
